As we get older, it is good to know there are home support services available in the community that can help you to carry on living an independent life in your own home.

At healthy@home, our goal is to deliver high quality community-based and in-home support services to achieve the best possible quality of life for our clients.

 Through the Commonwealth Home Support Program (CHSP), we can provide a range of services to support people aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people. The CHSP is a government-funded program for older people who need some extra help around the home to maintain their independence. The program places a strong focus on activities that support independence and social connectedness and take into account each person’s individual goals and choices.

 Healthy@home delivers a range of services, these include:

Nursing

Nursing care is clinical care provided by a registered or enrolled nurse. Through the Commonwealth Home Support Program (CHSP), our nursing staff can provide short-term or episodic treatment and monitoring of medically diagnosed clinical conditions.

Our nurses can provide advice on practices that help you or your relative to maintain good health and can offer treatments and care that improve capacity to self-manage pre-existing conditions.

We can provide a range of home nursing services such as: monitoring health care, post-operative care, help in managing chronic illnesses, general support for seniors.

Personal Care

The inability to perform activities of daily living (ADLs) can have a profound impact on your life, including your emotional wellbeing, so personal care specifically tailored for seniors can help you to maintain your independence.

Everyone is different and requires different types and levels of care.

Personal care includes a broad range of options, including:

  • Washing or showering
  • Toileting
  • Dressing
  • Incontinence care
  • Personal hygiene
  • Grooming
  • Assistance with eating
  • Assistance with mobility

Allied Health and Therapy Services

Allied health and therapy services focus on restoring, improving, or maintaining older people’s independent functioning and wellbeing. These services assist older people to regain or maintain physical, functional and cognitive abilities which support them to maintain or recover a level of independence, allowing them to remain living in the community. A range of therapies are available to help you maintain movement and mobility, including, but not limited to—speech  therapy, podiatry, physiotherapy, exercise physiology, occupational therapy and other clinical services such as hearing and vision services.

Domestic Assistance

A range of domestic assistance is available to ensure older people maintain their capacity to manage everyday activities in a safe, secure and healthy home environment. It usually refers to support such as helping clients keep their home clean, help clients with meal preparation, shopping, light gardening, clothes washing and more. Personalised services are available to address each client’s needs.

    Social Support

    At healthy@home, we offer our clients a range social support services to enable them to stay connected to their community.

    Keeping socially active is a vital part of maintaining good physical and mental health. We are inherently social and staying part of our community is fundamental to allowing us to continue to thrive as human beings.

    A range of individual and group social support for seniors is available including:

    • Organising home visits of family and friends
    • Assistance with transport to and from social gatherings and appointments
    • Connection with group activities, including transport provision, such as a group lunch or visit to a local club
    • Support with meal preparation, including shopping for food, advice from a dietitian or having regular meals delivered to the home.

    Transport

    Transport services refers to the provision of a structure or network that delivers accessible transport to eligible clients ensuring people to remain actively connected with their local community. Transport services aim to assist clients to continue with their usual activities, such as attending community groups or medical appointments, enabling them to keep active and stay socially engaged.

    Transport can help by:

    • Arranging a driver service
    • Providing transport vouchers and subsidies
    • Assistance with shopping, visiting health practitioners, and attending social activities

     Specialised Support Services

    This service type refers to specialised or tailored services for older people who are living at home with a particular condition such as dementia or vision impairment. These services can help you or your carers and families, to manage these conditions and maximise your independence to enable you to remain living in your own home.

    Some specific services may include—continence advisory services, dementia advisory services, vision support services, hearing support services and other support services such as occupational therapy driver assessments.
